In order to efficiently finalize your request, please complete the 'New Mustang Contact Information' tab found on your Orientation reservation portal prior to the submission of your request. Changes will not be made until it has been addressed. You may access your account under the 'Orientation Events' tab within your Cal Poly Portal (sign in at: my.calpoly.edu) or refer to your Admissions checklist.

*All requests must be made by their respective deadline. Failure to submit request prior to their dates, will result in an immediate denied request. This includes refunds and the request to be exempt from an event due to an extenuating circumstance that will be approved by the director.*

WITHDRAWN STUDENTS:
Students that have withdrawn their application from California Polytechnic State University, must complete this form to seek the respective refund. A processing fee of $50.00 per student will be withheld from the total reservation fee refunded. Refunds will be processed weekly, and will be returned to the student within 14 business days. Refunds will either be processed electronically to the original form of payment or by check.

RESCINDED STUDENTS:
Orientation will fully refund rescinded students automatically to their original form of payment. Our department receives daily reports, and will process the refunds weekly. If you have questions or would like a receipt of the refund, please contact orientation@calpoly.edu. If our department does not have the student's information by the time you are requesting this information, please contact admissions@calpoly.edu.

RETURNING STUDENTS:
If you wish to not participate in either orientation event as you have previous gone through the process, please contact your College Advisor and request a written exemption from the event(s) and forward the correspondence to orientation@calpoly.edu.

Each type of request will require you, as the student or the supporter to complete the request and agree to these terms. This includes our office staff to process your request within 14 business days from the moment you have submitted the form. Supporters completing this request must have your student's Cal Poly full name and email address available to proceed with this form.
