Registration Form - Library Access for Nonprofits
Thank you for your interest in this workshop for regional nonprofits, offered by Appalachian State University Libraries! Please complete this form so that we may prepare workshop content to meet your needs and interests. 
  • Lunch as well as morning and afternoon refreshments will be provided. 
  • Passes for on-campus parking will be provided. 
  • After we receive your registration form, we will contact you about your dietary needs for lunch and refreshments as well as any accommodations. We want to ensure that participants have a rewarding experience and are able to participate with ease. Please let us know how we can accommodate your needs with regard to accessibility, mobility, or any other issues.
  • You are welcome to bring your own laptop. However, if you need a laptop, please let us know, and we will be happy to provide one for you.
  • Before the workshop, we will provide participants with parking passes, directions to campus, and other workshop details.
  • The workshop will be held in room 421 of Belk Library and Information Commons on the campus of Appalachian State University. 
  • Registration is limited to 20 participants.
