I represent that I am the parent, legal gradian, and/or representative of accompanying minor (hereinafter "Child(ren)"). If I am not the parent or legal guardian of any Child(ren), I expressly represent that I have the permission of the Child(ren)'r parents(s) and/or guardian(s) to act as their agent in signing the waiver and release.

In consideration of participating in Touch-A-Truck (hereinafter "TAT"), sponsored by the Junior League of Corpus Christi, Inc. on Saturday, March 19, 2022 and Whataburger Field south lot in Corpus Christi, Texas, I agree to the following on behalf of myself , the Child(ren), my executors, administrators, heirs assigns, and successors:

1. Waiver and Release of Liability
I expressly understand that participation in TAT is at my and the Child(ren)'s own risk. I hereby forever release, discharge and hold harmless the Association of Junior Leagues International and the Junior League of Corpus Christi, Inc. and all of its agents, volunteers, representatives, employees, and trustees (collectively, hereinafter "JLCC") from any and all liability, claims, demands, actions, and causes of action arising out of or related in any way to any damage, injury, expense or loss of any kind, including but not limited to any personal injury, death, and/or property damage, resulting from or related to (a) TAT; (b) any action or omission of the JLCC, whether intentional or negligent during TAT; (c) any action or omission by me and/or the Child(ren), heather intentional or negligent during TAT; and (d) the publication of the likeness of image of me or the Child(ren) that is captured and/or recorded during TAT.

2. No Duty to Supervise
I acknowledge that a parent or guardian MUST accompany and supervise the Child(ren) participating in TAT. I agree and understand that the JLCC has no duty to monitor, control, or supervise me, the Child(ren), or anyone else during TAT. I agree to fully monitor, control, and supervise the Child(ren) during their participation in TAT, and that I am solely responsible for any injuries sustained or caused by the Child(ren) during TAT.

3. Minor Photo Release
I give the JLCC, its sponsors, vendors, agents, and subcontractors, and (b) The Association of Junior Leagues International, Inc. permission to publish in print, electronic and/or video format the likeness or image of the Child(ren) that is captured and/or recorded during TAT.

By acknowledgement below, I agree that I have carefully read this waiver and release, fully understood it's terms, realize the at I am giving up substantial legal rights that I and the Child(ren) have, and sign it freely and voluntarily
