As the guardian of the above-referenced dog(s), I understand Cross Road Boarding, Training &
Behaviour (herein known as “Cross Road”), its employees, officers, directors, and agents will
exercise due care to protect the health and safety of my dog(s) while in their care. In the event
my dog(s) becomes ill or sustains injury I have given my permission for those in charge to take
whatever steps are necessary to obtain medical treatment for my dog(s), and I agree to pay all
charges incurred. I consent to any veterinarian being retained to render care for my dog(s).
I understand the concept of dog daycare/boarding/training/transport services and that there is a
chance of injury, loss and death of my dog(s).
If my dog(s) are to be picked up by another person, I understand that I must make Cross Road
staff aware of this and either a) prepay for any services rendered or b) ensure payment is able
to be made at pick up. I understand that if I do not arrange for payment, then my dog(s) will not
be released until payment has been received. Charges for services will continue to incur until
the balance is paid in full. Prices are subject to change at any time without notice.
By leaving my dog(s) at Cross Road, I agree not to file legal charges against their employees,
officer, directors, or agents for any injury, death, or loss of my dog(s). I hereby waive and
release Cross Road, its employees, officers, directors, or agents, from any and all liability of any
nature, for any injury, death, or loss of my dog(s) resulting from Cross Road actions or the
action of my dog(s) or any other dog(s) while in the custody of, on the grounds or surrounding
area of Cross Road.
In the event my dog(s) causes injury to another dog(s) or to a person while at Cross Road, I
agree to indemnify Cross Road from any action which may be brought against it, and for any
defence settlement or judgment against it I will assume all liability for the actions of my dog(s)
and agree to maintain personal liability insurance to cover me in the event of such an incident. By entering your name below, you agree to the above terms and conditions for the dog(s) listed on this form.
