The purpose of this form is to report Information Security concerns regarding people, processes, data, and technology to which Grand County is the custodian of.

The data within this form is reported directly to the Grand County Chief Information Security Officer (CISO) who is responsible for researching, determining the appropriate next action to ensure the protection of information and assets.

The Grand County CISO will maintain the anonymity as requested; provided such anonymity does not conflict with any laws and regulations.
