Volunteer & Partner Screening and Badge REQUEST Form 2023-2024
Please complete this form to begin the process of having the Department of Safety and Security run a background check that will be used to designate eligibility for the 2023-2024 school year.

IMPORTANT: Even if you were approved any previous year, annual renewal is required - the background check must be requested again if more than 4 months old *** Badges issued expire and must be renewed ***

NOTE: THIS PROCESS IS NOT INTENDED FOR CURRENT APS EMPLOYEES WORKING IN NON-APS PROGRAMS

In addition to completing this online request form, a copy of your ID and a current picture is also required and must be submitted at the school/site where you will be performing work. That is a required step that needs to take place in order for your request to be considered.

NO REQUEST WILL BE CONSIDERED UNTIL AN APPROVAL IS SUBMITTED FOR THE APPLICANT COMPLETING THIS FORM. 

Immediately after you finish completing this form, the confirmation screen will show a link to Sterling Volunteers and the Good Deed Code you must use (aps301v) in order for us to be charged for the cost of the background check. You MUST complete the background check information to consider the badge request process complete. But know that no one will contact you and your request will not move forward until the school submits your approval. 

                                                APS no longer has a COVID Vaccine Requirement.

Badges will be printed after background check results are reviewed and approved to make you eligible, and after it is confirmed by the school that they are approving you as a volunteer for the current school year. 

All badges will be sent to an APS site for pick-up; no exceptions.

WE CURRENTLY DO NOT APPROVE VOLUNTEERS AT CHARTER SCHOOLS ASSOCIATED WITH APS.
