Oxford Brookes Touch Rugby 2020/21: Self-Declaration to Participate

COVID-19 self-declaration form to participate in Oxford Brookes Touch Rugby activities.

This form must be fully understood and completed before attending any training sessions in Oxford Brookes University Centre of Sport Astroturf at Headington. You will be required to complete a new one of these forms before every session.

You will not be able to attend if you have not complete one of these forms. This form is to ensure the risk associated with the transmission of COVID-19 is reduced and to enable track and trace in the event a player should develop symptoms.

Symptoms of COVID-19 include:

- Persistent dry cough
- Persistent high temperature
- Persistent loss of taste and/or smell

If you develop symptoms within 48 hours of attending a session, it's your responsibility to inform the committee members of the club:
