SGAH Adoption Application
Please complete the following questions to the best of your ability. Once submitted, our adoption director will get back to you within 48 hours. 

To be considered as an adopter the following requirements must be met: 
  • Be 18 years of age or older 
  • An application and a personal interview are required
  • All pets in the home must be up-to-date on vaccines, heartworm prevention, and spayed or neutered
  • Previous pet ownership and a veterinary history is preferred but not required for all pets
  • Priority will always go to applicants with previous breed experience 
  • Have knowledge and consent from your landlord 
  • Be able and willing to spend the time and money necessary to provide medical treatment and proper care of the dog and/or cat 
  • Have reliable transportation to pick up pet and a valid driver's license 
  • The adopted pet must live indoors at all times and be treated as a member of the family 
Decisions are made at our discretion, we reserve the right to chose the best parent for each particular pet. We highly rely on the pet to parent interaction to chose our adopters.
