By signing below, in consideration of my child being permitted to participate in the Ellen Browning Scripps – Academic Fundraising Partnership (EBS-AFP) sponsored Morning Run Club (MRC), I hereby agree to the following:

1. I recognize the potential risks associated with my child participating in the MRC. I understand that potential risks include physical exertion that may cause physical injury.
2. I certify that my child is in good health and has no medical condition that makes participating in the MRC risky for my child. By signing this, I have been advised to consult with my child’s doctor regarding my child’s participation in the MRC.
3. I understand that I am required to provide adult supervision for my child at all times during his/her participation in the MRC. MRC IS NOT a student drop-off program. I understand that supervision WILL NOT BE provided by the parent volunteers and/or volunteer staff that may be present before, during or after each run event.
4. I agree to assume full responsibility for any risks and injuries, known or unknown, which my child may incur as a result of his/her participation in the MRC. MRC will not provide any medical care related to any injury that occurred while my child is participating in the MRC. I understand and agree that I will be responsible for any medical costs associated with any injury sustained arising out of my child’s participation in the MRC.
5. I understand that MRC is not sponsored by or authorized by San Diego Unified School District and/or Ellen Browning Scripps Elementary School.
6. I knowingly, voluntarily and expressly agree, on my child’s behalf, my own behalf, and on behalf of my child’s and my heirs, assigns, executors, administrators and legal representatives, to forever release, waive, hold harmless and discharge MRC, the San Diego Unified School District and Ellen Browning Scripps Elementary School, and their directors, officers, employees, agents, volunteers, representatives and assigns, from and against any and all claims for personal injury, property damage and death (including costs and expenses, such as attorneys’ fees and costs) arising directly or indirectly out of or relating to my child’s participation of the MRC, including without limitation where the injury, damage or death is caused by MRC’s negligent acts or omissions.
7. I understand and agree that the MRC may terminate my child’s participation in the MRC at any time in its sole discretion. Students that create an unsafe environment may be asked not to participate.
