Personal information of RIS students, parents, and guardians is on file in PowerSchool as a matter of academic record and as a familial reference about the well-being of the student.

New students (new to RIS) who have recently completed the admissions and registration process do not need to complete this form.

Student health data required by the RIS School Clinic will no longer be included in this form. Please download the documents linked below, complete them, and submit them to the clinic or section office by August of the new school year.   
1. Annual Clinic Update Form (formerly School Clinic Card) 
2. Hospital Check Up Data Form (for incoming RIS students and rising Grade 6 and 9 students only) 

If you are unsure of the information the school has, we encourage you to fill out the form anyway. Your information will be cross-checked accordingly. If there is any information you would like to change that is not on this form, please contact advancement@rism.ac.th
