Covid Rapid Test Kit Request
Please complete the following form in order to recieve rapid test kits.
Kits (5 tests/kit) will be dispersed one box at a time to families upon request, as long as we have supplies at the school.
We ask that families only request kits as they are needed since supply issues in the school are limiting availability. There are many community based sources for tests as well as those in the school. Please consider clicking the link below to take you to sites where tests are available.
