South Valley Community Church Facility Use Request Form

Thank you for your interest in utilizing our facility!

Please fill out completely and return to the church office electronically. Your reservation will be confirmed upon receipt of this form, payment of deposit if required and approval from a member of our staff. DO NOT ADVERTISE YOUR EVENT WITHOUT APPROVAL.

General Fees:
Deposit to reserve the date: $100*

Worship Center
Members: $500
Non-members: $1,000

Chapel
Members: $350
Non-members: $700

Technical Crew: $40 per hour (two hour minimum)
Custodial/Setup Crew: $30 per hour (two hour minimum)
Utility Fees (June-Sept only): $150
Musicians (if needed to be provided by SVCC): $150 per musician

*Deposit is subtracted from the total amount.

Please note: You are responsible for setting up, cleaning up, and returning all items to their proper place. Each group is responsible for providing their own paper products. SOME ITEMS ARE NOT AVAILABLE FOR NON-CHURCH FUNCTIONS. If you request to use the Worship Center and need to use the sound or tech equipment, you MUST use one of our trained technicians.  A fee may be assessed for their compensation.
