The Utah Democratic Party is committed to ensuring our internal and proprietorial list of personal contact information is secure and used only for campaign and/or organizing purposes by those with permission to access.

If a campaign is making the request, the candidate must complete this form and understand they are responsible for the adherence of these terms. Candidates will only receive access to the portion of their list that corresponds with the district in which they are running.

As with all Party data, acceptance of this list means that you agree to the following conditions on its use:

1. The list is for you to use for your campaign/organizing purposes ONLY

2. The list should not be shared with others, except on an as needed basis for campaign/organizing purposes. YOU are responsible for ensuring the list is used via these terms & conditions

3. Copies of the list should be destroyed when of no further use to your campaign/organizing purposes

4. Robocalls with this list are strictly prohibited as it could violate Federal Law (cell phones)

5. Email and other mass communications should be sent in a way that ensures the privacy of recipients. For example, please use a listserv or the bcc function whenever you send group emails
