CBSL Little League Incident Notification Form
The Caledonia Baseball Softball League needs your help to keep our league, facilities and play as safe a possible. We ask that this form be filled out as accidents, near missing and incidents happen throughout the season, practice or play. After completing this form, our Board President, Mallory Walsh, will be in touch to review and help all parties remedy the unfortunate situation. 

This form must be completed by parents (if claimant is under 19 years of age) within 5 days after the accident.
