1. By typing my full name below before submitting, we have been informed and understand that the Cleveland Cultural Gardens Federation is hosting the One World Day annual event, in which my name, likeness, image, and/or voice may be included.

2. I hereby grant Cleveland Cultural Gardens Federation, it’s volunteers, the One World Day team, employees and agents, the right to make, use and publish in whole or in part any recorded footage in which my name, likeness, image and/or voice may be included (hereinafter "Recordings") whether recorded on or transferred to videotape, film, slides, photographs, audio tape, digital format, or other media now known or hereafter developed.  This includes, without limitation, the right to edit, mix, duplicate, use or reuse Recordings as it may desire without restriction as to changes or alterations.  Cleveland Cultural Gardens Federation shall have complete ownership of the Recordings in which I or my performance or contribution appears.

3. I also grant Cleveland Cultural Gardens Federation the right to distribute, display, broadcast, exhibit, and market any of said Recordings, either alone or as part of its finished productions;  for commercial or non-commercial purposes as Cleveland Cultural Gardens Federation or its representatives and agents may determine.  This includes the right to use said recordings for promotion or publicizing any of these uses.

4. I hereby waive any and all right that I may have to inspect or approve the finished product or printed matter that may be used in connection therewith.

5. I expressly release Cleveland Cultural Gardens Federation and all persons acting under its permission or authority from any claim or liability arising out of or in any way connected with the above uses and representations including any and all claims for defamation or copyright infringement.

6. I understand that our participation and performance at the designated garden will be all volunteer basis, no monetary compensation will be provided.

7. I understand many of the gardens do not have electricity outlet, I will be bringing my own speaker (PA) system to play back music needed for my performance group.

8. I understand that there is no monetary compensation to participate at Cleveland Cultural Garden's One World Day event.

I am over the age of eighteen, and have read the above release, and fully understand its contents.
