PPMD Duchenne Healthcare Professionals Summit - Guest Hotel Reservation

This form will enable PPMD to reserve your hotel stay on your behalf at the Nugget Casino Resort in Sparks, Nevada.

Please include your name, contact information, date of arrival, and date of departure for the Summit. This will allow us to book the appropriate number of rooms via the room block at the venue.

INFORMATION SPECIFICALLY FOR SPEAKERS AND INVITED GUESTS FOR WHOM PPM WILL COVER LODGING COST: As the meeting runs from Wednesday 1/19 through Friday 1/21, PPMD will cover check-in Tuesday night 1/18 through Friday night with a Saturday 1/22 departure. Exceptions include those who are attending the nursing pre-conference, who will arrive Monday night 1/17 for the Tuesday meeting. You are welcome to book additional nights at your own expense if you wish to arrive early or depart late.
