I hereby give consent for my child(ren) to be photographed, videotaped, audio recorded, or display/broadcast in online short films created by my child(ren) or other children in the STAR program and submitted by STAR staff for purposes of publication, display or broadcast (print, web, digital display and all other forms of media), promotion and marketing of the STAR Education "It Was a Dream" Short Film Online Competition.

I agree that such materials are the property of STAR and I relinquish any present or future claim for reimbursement for said photographic or film reproduction.

I hereby release STAR, Board of Directors, its affiliates, employees, and representatives from any and all claims, demands, costs, and liability that may arise from the use of the above-mentioned materials.

I acknowledge that I have read and understood this consent form in its entirety and have checked the appropriate box below. I understand and I have the option of opting out by not giving consent.
