This form will be used to apply for enrollment into the program at Woodland Park Academy. Please complete all portions of this form so that staff can take the next steps to get your child(ren) enrolled.

Please note that Woodland Park Academy will also need a copy of the student's birth certificate, immunization records or waiver, and a copy of the guardian's Drivers License or State I.D. This will all need to be turned in before the student can attend school. Thank You!

Once the office staff receives your application, they will send you a packet and a letter in the mail. Please fill out and include any missing information and send it back in the self-addressed stamped envelope as soon as possible.
