Thank you for your desire to submit an elder nomination! We are grateful for your desire and investment to ensure faithful, godly leadership for our church. 

Please read the following notes before completing the form:

1) Only covenant members of New Hope Church have the privilege of nominating elders for New Hope. Nominations will be verified to ensure they are completed by covenant members.

2) Please remember that these nominations should be completed WITHOUT the knowledge of the man who is being nominated.

3) Please note that this nomination process will take significant time to complete. There is no way to save your work and come back at a later time. You may want to review the application to ensure you can complete it fully before you begin. For the questions requiring written responses, you may want a separate document and then copy & pate your answers to this form when you're ready to submit. 

4) Please answer all questions. If you need to provide any clarification or comments to explain your selections, please utilize the open fields at the bottom of this nomination form. 

5) If you do not know the nominee in any specific area asked about below, please answer "DO NOT KNOW" rather than make an assumption. This will not be held against the nominee.
