This form must be completed by the player.

This membership entitles the player to:
- 3 complimentary training sessions with Sandbach Hockey Club, to be taken at any time.

After 3 sessions, you must become a club member before participating in any futher sessions.

By completing this membership form you confirm that the player will be bound by the player code of conduct. Failure to follow club regulations may result in the cancellation of membership.
