Hosted by Southminster Presbyterian Church in partnership with the Crestwood Police Department 


Saturday, June 24th, 10am-2pm

Bike riders of all ages are welcome

Bring your helmet, bike, and water bottle

11am neighborhood bike ride for younger elementary kids

1:30pm bike ride for older middle school youth

Registration required – Parents must sign permission/waiver form either in person or online

10 restored Bikes were donated by the Mehlville Lions Club for Kids in Need. If you want your child to be considered for a bike (or to exchange a bike for a different size) OR if you have an extra bike you would like to donate, please contact southminsterstlouis@gmail.com.

Agreement to release and hold harmless the City of Crestwood and/or Southminster Presbyterian Church. Without this top line, the document refers to the city and church without identifying them until near the end.

Please read this section carefully and be aware that in participating in the City's/Church’s Program(s), you are expressly assuming the risk and legal liability and releasing all claims of injuries, damages, or loss which you, or your minor child, might sustain as a result of participating in any and all activities associated with the Program(s). By signing this Agreement, you as an adult, or as a parent or legal guardian of a minor, are acknowledging that you are aware of the risks associated with the Program(s) and that you are freely assuming those risks.

In exchange for the City sanctioning the Program(s), providing City-paid staff members to take part in the Program(s), and permitting my participation in the Program(s), I agree as follows:

I hereby acknowledge that I understand the nature of the Program, and believe that I am, and/or my child(ren) are, qualified and in proper physical condition to participate in the Program. I further agree that if at any time, I believe participation in the Program is unsafe with respect to my or my child(ren)’s physical condition, the equipment, or facilities, it shall be my responsibility to immediately discontinue participation in the Program.

I further acknowledge and agree that I am aware that participation in the Program involves the risk of illness, serious bodily injury (including the possibility of permanent disability, paralysis, or death), which may be caused by (a) my, or my child(ren)’s, own actions or inactions, (b) the actions or inactions of others participating in the Program, (c) the condition of the equipment and/or facilities at which the Program is located, or (d) the actions or inactions of the Releases identified below; and I fully accept and freely assume all such risk and all responsibility for losses, costs, and damages I and/or my child(ren) may incur as a result of participation in the Program.

I, for myself, my child(ren), and my personal representatives, release, forever discharge, and waive the right to pursue any and all claims against, Southminster Presbyterian Church, the City of Crestwood, its Board of Aldermen, agents, officers, employees, and others acting on the City's or Church’s behalf (collectively, "Releases”), of, from, and against any and all claims, demands, causes of action, suits, and/or legal liabilities for any personal injury, death, damage or loss to property, liability or expense, including attorney's fees, arising out of the below-named participant's participation in the above Program(s), even if such damages, injuries, or death are alleged to be in whole or in part, directly or indirectly, the fault of or caused by the negligence or carelessness of the Releases, including, without limitation, claims arising out of the dangerous condition of City property, the negligence of City employees, or a COVID-19 infection arising before, during, or after participation in the Program.


I, for myself, my child(ren), and my personal representatives, agree to defend, indemnify and hold the Releases, harmless from and against all claims for personal injury, death, damage or loss to property, liability or expense, including attorney's fees, arising out of the below-named participant's participation in the Program(s), notwithstanding any possible negligence, (whether sole, concurrent or otherwise) on the part of the Releases.

Photo Consent:
You have received this parental consent form to both inform you and to request your permission for your child’s photo/image and name to be published on southminsterstlouis.org and/or any other social media sites maintained, owned, and/or administrated by Southminster Presbyterian Church.

The law requires that we ask for your permission to use information about your child. Pursuant to law, we will not release any personally identifiable information without prior written consent from you as parent or guardian. Personally identifiable information includes youth names, age, grade, and photo or image.

If you, as the parent or guardian, wish to rescind this agreement, you may do so at any time in writing by sending a letter to the Ruling Elder of Christian Education, Lee Howard-Ginther and such rescission will take effect upon receipt.
