PRESCHOOL to GRADE 5 TEACHER COLLECTION REQUEST FORM
If you would like a staff member to collect books, audiobooks, or movies for you on a particular theme, subject, or author, you will need to fill out this Teacher Collection Request form TWO WEEKS IN ADVANCE of when you want to pick-up the books.  We often need to request materials from other libraries in our consortium, and this could take up to TWO WEEKS. Therefore, the more lead time you give us with your request, the better your chances of getting a good selection of materials.
