Open Minds' Under 18s gender identity research form 
Thank you for filling in this form - Open Minds is a counselling service in Doncaster who often support children, young people and adults who are trans or gender non-conforming. 

We are trying to find out what under 18s in Doncaster would like to see in terms of support around their developing gender identity and gender expression. 

This does not mean we will be able to meet the needs that are expressed in this form, but we will certainly try and talk to the right people both in Doncaster and beyond who can help. 

The feedback you give will be collated and kept anonymous, but there is an option at the end of the form to include your contact details if you would like to be kept informed about what we are doing and as much as we can we will keep you inform about what else is happening in and around Doncaster to support children and young people with their gender identity and gender expression. You can withdraw permission for this at any time. 

We are particularly interested in the ways in which gender non-conforming young people may need different support, and want to find out as much as we can in order to work out what support young people: 

1) need / want 
2) would be willing or able to access 
3) would struggle to access, for example if their parents don't know their true gender identity

A brief word about terminology 
We understand that some of the words in this form might be confusing, and some of the questions might even be upsetting.

We have used the term TRANS to refer to people who identify differently to the gender they were identified at birth. 

We have used the term CIS to refer to people who identify the same as the gender they were identified at birth. 

We have used the term GENDER NON-CONFORMING to refer to people who express their gender differently to what might be considered 'mainstream' gender expression.
