CONSENT FOR ENROLLMENT IN TEMPORARY
CHILD CARE PROGRAM AND RELEASE OF LIABILITY

PLEASE READ THOROUGHLY BEFORE MOVING ON TO THE NEXT SECTION.

I am the parent or legally authorized representative of the child named below. By submitting
this form, I am consenting to my child’s enrollment and participation in the temporary child care program (the “Program”) being offered by True Texas Project - Denton County (“TTP-DENTON”).

I understand that TTP-DENTON is not licensed by the state of Texas as a child
care provider, but is offering this Program to its attendees at no charge.

A FEW HOUSE KEEPING RULES:

1) Maximum time allowed per day: 2 hours
2) Parent/guardian must remain on premises while child is in the care of TTP-DENTON volunteers.
3) For the well-being of the other children, please refrain from bringing a sick child into the childcare area.
4) No food or drink will be permitted in the childcare area.
5) Disruptive or inappropriate behavior will not be tolerated. In these circumstances, parents will be notified immediately.
6) If a child is inconsolable for more than 15 minutes, parents will be notified and asked to return to the childcare area to assist staff.
7) Medications will not be administered by childcare volunteers.
8) Please keep your child freshly diapered. We do not provide changing services. If a diaper change is needed, parents will be asked to return to the childcare area and change the diaper.
9) Please label all of your child's belongings with his/her name.
10) the child room will provide age appropriate toys. For hygienic reasons, we ask that children refrain from bringing their own toys to the childcare area.

I understand that TTP-DENTON will release my child only to me or to the individual(s)
who I specifically authorize (including another parent/legally authorized representative of
the attendee). I will inform the individual(s) identified below that they may be required to show photo identification to verify their identity before my child will be released to them.

I, the undersigned, hereby hold harmless, waive and release TTP-DENTON, their childcare workers, officers, representatives, agents, organizers and successors from liability as a result of personal injury or property damage occurring while the above child/ children is/are in their care at TTP-DENTON. I understand that this is not a licensed childcare facility. I understand that childcare services are provided only while I am present in the building and training. I understand that childcare services are provided without charge by volunteers. I understand that if my child should become inconsolable during the event, I am responsible to leave the meeting and attend my child. I have read, understand and agree to adhere to the Childcare Policies and Procedures of TTP-DENTON listed above.  .

THIS IS A LEGAL RELEASE OF LIABILITY AND CONSENT FORM. I HAVE READ
THIS FORM CAREFULLY AND HAVE HAD ALL QUESTIONS ANSWERED BEFORE
SUBMITTING. BY SUBMITTING THIS FORM, I HEREBY REPRESENT THAT I HAVE THE
AUTHORITY TO PROVIDE CONSENT ON BEHALF OF MY CHILD.
