Care Basket Requests

Use this form to request Care Baskets for yourself or someone you know that would benefit from one. Care baskets contain a meal and an assortment of fun snacks! All items are vegetarian and in commercially sealed containers. No need to tell us who the Care Basket is for.

Instructions for picking up care baskets

Go to CSE1 (Allen) 386 ("behind" the elevators) and turn right when you walk in. Note that you must have your physical key for the Allen building to enter the office. If your recipient has dietary restrictions, please feel free to sort through the contents and remove/distribute anything your recipient cannot eat themselves.

Please submit one request per care basket needed so that we can keep track of how many we have!
