Dear Parents & Guardians,

Please complete the form below regarding your transportation needs for the 2022-23 school year.  Should the need arise to revise your plans please submit another form or contact the Transportation Office (716.542.5026).  If you have more than one child with the same needs you may use one form.  If your children have different needs you must complete one form per child.

This form is for elementary students only.  If you have students in middle or high school, please visit  https://forms.gle/ZEtrXGZQjar6pgoYA to complete the appropriate survey.

Reminders:
~If any changes arise during the school year please complete this form again or send a note to the office with your child.  Please be sure to include your child's first and last names on the note.

~All daily bus notes and changes MUST be received in the Elementary School Office (716.542.5050) prior to 2PM.

~A responsible adult must be visible to the bus driver for students in kindergarten, first and second grades when returning home or to another location.  Parents allowing older siblings to supervise younger siblings must have approval of the elementary principal or assistant principal.  If there is not an adult visible your child will be returned to the Elementary School Office where you will be contacted to pick them up.
