By completing this form, I give permission for my child to participate in the 2023 Box Elder Team Football Camp.  I acknowledge the risks involved in football and I will not hold the camp staff liable for any injury.  I give permission to the coaches and trainers involved in the camp to coach and teach my son the fundamentals of successful football. I also give them permission to treat my son in case of injury or illness related to the camp.
