Financial assistance requests made on Thursdays or the weekend will be reviewed the following week. Incomplete information may result in a delay in processing.
After you submit your application email us the following documents. Your name and date of birth must appear in the subject line of your email. Please try to send everything in one email.
1) a copy of your most recent tax return page 1 and 2 of the IRS form 1040; and
2) the name, contact number for current or most recent last employment and last 3 paystubs, and
3) copy of any eviction or disconnect notices you may have received, and
4) Proof of employment
5) A copy of your lease
6) A copy of you utility bill if seeking assistance for utilities
If you are making a claim that your circumstances are COVID-19 related, we need proof of job loss or reduction in hours. You can do this by submitting multiple paystub screenshots for consecutive weeks or a letter from your employer.

If you are staying in a motel or hotel in Irving with children in Irving ISD or Carrollton-Farmers Branch ISD, in addition to the form you just completed, email the full names of your children and the schools they attend.
