MEAL ORDER FORM FOR TUESDAY, SEPTEMBER 22nd and THURSDAY, SEPTEMBER 24thTH PICK-UP                                  
Tuesday pick-up (3 Breakfast and 3 Lunches per child) and Thursday pick-up (3 Breakfast and 3 Lunches per child).  Form must be completed no later than Sunday at 3 pm.

The Schuylkill Valley Food Service Department will be offering free grab and go meal service for all children between the ages of 2 and 18 who reside in the Schuylkill Valley School District.  Meals will be provided for instructional days only and will not be provided for holidays and in-service days in which school is not in session for students.

ALL MEALS WILL BE DISTRIBUTED FROM THE HIGH SCHOOL.  Meals will be distributed every Tuesday (3 breakfast and 3 lunches provided) and Thursday (3 breakfast and 3 lunches provided).  Pick up time is between 3:30 pm and 5:00 pm.  Meals will be distributed at door number 9 which is located behind the building near the tennis courts.  Signs will be posted to direct you to the pick-up location.  MEAL DELIVERY SERVICE WILL NOT BE PROVIDED.

Families who would like to participate in the grab and go meal program must pre-order the meals on a weekly basis.  The order form will be available on the district website between 1:00 pm Thursday and 3:00 pm Sunday.  Please fill out a form each week between those times in order to receive meals for the following week.  ONLY COMPLETED FORMS WILL BE PROCESSED!

PLEASE DO NOT ORDER MEALS IF YOU CANNOT PICK THEM UP!!!!!!
