I fully understand & acknowledge that the use of equipment involves inherent risks, which may result in injury, including permanent disability & death. I consent that Haven Wellness is not liable & held harmless for any injuries sustained while using Infrared Sauna, Halo Therapy Sauna, Cryo Therapy Chamber, & Oxygen Bar equipment.
