Families who qualify for district-provided bus transportation must fill out this form to denote their children will ride the bus for the 2020-21 school year. This information will be used to update routes for the coming school year.

As a reminder, a student is eligible to receive district-provided bus service if his or her home is 2 or more miles from school via the shortest route (in terms of distance) using public roads. Please be aware that elementary students may have to walk up to .25 miles and secondary students may have to walk up to .5 miles to a bus stop. Additionally, students can receive transportation if they live along a hazardous area (see www.ldisd.net/hazardous). If your child has special needs, please contact your school principal to determine if transportation is included in his or her plan or should be added.
