Thank you for your interest in delivering Faithful Families in your local community this year!
This form is intended to give us more information about your plans for starting a Faithful Families program in your community. The first step to implementation is completing the Faithful Families Moodle Training and annual live virtual training (or individual one-on-one call with Kim Eshleman). If you have completed these training steps and are ready to start a program, please complete the form with as much information as you are able. Once you have submitted, we will follow up to talk through next steps, and connect you with the resources that you need and find out how we can best provide ongoing support for your efforts.
If you have not completed training, please contact Kim Eshleman (keshlem@ncsu.edu) for more information about completing training before you begin implementing the program. If you are new to Faithful Families, this is a great way to get started. If you have previously attended training and/or implemented the program, please feel free to use the online training and resources as a refresher and for updated content. Once you have completed these steps, we will reach out to talk through next steps and completing this program startup form.
