4th Quarter Enrollment Survey
This form only needs to be completed if your child will be moving from remote learning to in-person learning or vice versa for the 4thQuarter.  If your child's status will remain the same the form does NOT need to be completed. Please complete by Thursday, March 11.  The 4th Quarter begins on Monday, March 15.

For my student, I choose to have him/her __________ with the understanding that in-person instruction will require students to wear masks and that social distancing will be enforced as much as possible, AND the understanding that remote learning will be substantially more rigorous and thorough compared to the remote-learning process last spring.

In the light of the need to reduce the number of students on a bus, the District will only be transporting students who live 1.5 or farther miles from the school or would have to cross a hazardous highway.  The District will NOT be providing transportation to students living in town unless they would have to cross a hazardous road.
