Armada Area Schools is hosting two COVID-19 Pfizer vaccination clinics for individuals ages 12 and up. The date for the first dose is Monday, August 23rd (11 AM to 3 PM); the date of the second dose is Monday, September 13th (11 AM to 3 PM). Registration is not required, but by providing your contact information, we will be able to make sure we have enough vaccine available.

Children ages 12-16 years old must be accompanied by an adult relative who knows the medical history of the child and must have signed parental permission on the consent. Students who are 17 years old do not need an accompanying adult but do need the parent consent part of the form filled out. Individuals age 18 and up are consenting adults and should fill out the consent form for themselves.
