Regular physical activity is fun and healthy, and increasingly more people are starting to become more active every day. Being more active is very safe for most people. However, some people should check with their doctor before they start becoming much more physically active. If you are planning to become much more physically active than you are now, start by answering the seven questions below. If you are between the ages of 15 and 69, these questions will tell you if you should check with your doctor before you start. If you are over 69 years of age, and you are not used to being very active, check with your doctor before increasing your activity. Common sense is your best guide when you answer these questions. Please read the following questions carefully and answer each one honestly: check YES or NO. *
Has your doctor ever said that you have a heart condition and that you should only do physical activity recommended by a doctor? *
Do you feel pain in your chest when you do physical activity? *
In the past month, have you had chest pain when you were not doing physical activity? *
Do you lose your balance because of dizziness or do you ever lose consciousness? *
Do you have a bone or joint problem (for example, back, knee or hip) that could be made worse by a change in your physical activity? *
Is your doctor currently prescribing drugs (for example, water pills) for your blood pressure or heart condition? *
Do you know of any other reason why you should not do physical activity? *
Here comes all the legal jargon!

IF I BEGIN THE PROGRAM, I (the first-person is hereinafter understood to be the Client, listed above) hereby agree to participate in the health and wellness program designed by “ThinkWin Holdings, LLC” or its assigns and agents (together d/b/a LeMasters Fitness and together hereinafter referred to as LeMasters Fitness). I understand that this program may include exercises to build the cardiorespiratory system (heart and lungs), the musculoskeletal system (muscle endurance, strength, and flexibility), and to improve body composition (decrease of body fat in individuals needing to lose fat, with an increase in weight of muscle and bone). Exercise may include aerobic activities, calisthenic exercises, and weight lifting to improve muscular strength and endurance, and flexibility exercises to improve joint range of motion. The program may also include a wide variety of suggested lifestyle habits coordinated to improve overall health and wellness based on my individual needs and goals. I also understand that some or all of this program may involve remote or online coaching and guidance.

I understand that the reaction of the heart, lung, and blood vessel system to exercise cannot always be predicted with accuracy. I know there is a risk of certain abnormal changes occurring during or following exercise, which may include abnormalities of blood pressure or heart attacks. Use of the weight lifting equipment, and engaging in body calisthenics may lead to musculoskeletal strains, pain, and injury if adequate warm-up, gradual progression, and safety procedures are not followed. I understand that LeMasters Fitness shall not be liable for any damages or personal injuries I sustain as a result of (during or after) the program. I participate in the  program, using the associated equipment, at my own risk. I assume full responsibility for any injuries or damages that may occur during or after the training. I hereby fully and forever release and discharge LeMasters Fitness from all claims, demands, damages, rights of action, present and future therein.

I understand, warrant, release, and agree that I am in good physical condition and that I have no disability, impairment, or ailment preventing me from engaging in active or passive exercise or practices that will be detrimental to my safety, comfort, or physical condition if I participate in the personal training program (other than those items fully disclosed to Trainer). I understand that a physician’s examination is recommended for all participants with any exercise restrictions, all men 45 years and older, and all women 55 years and older. I acknowledge and understand that I have been informed of the importance of a physician’s examination. I accept full responsibility for my health and well-being and acknowledge and understand that no responsibility is assumed by LeMasters Fitness for any disease, limitations, or injuries, mental or physical, that LeMasters Fitness was unaware or uniformed of.

Your transformation results will probably be inspiring and we would love to show you off! By being participating in this training system, you agree to grant LeMasters Fitness the irrevocable right to use your image, likeness, photographs, video content, or audio recordings of you captured or shared online as part of our marketing and sales throughout the world and in perpetuity. You also release us from all claims you may have relating to the use mentioned in this section. We can also happily keep you anonymous and will do our best to accommodate any request you may have to stop using an image or other media of you.
