Instructions for Completing and Submitting this Proxy Ballot:
• By completing this Proxy Ballot, you are authorizing the Secretary of Delaware County Farm Bureau to cast your votes pursuant to your instructions as set forth below.
• For your votes to be counted, you must complete all the information at the bottom of this form, including your signature, printed name, date, and membership number. Your membership number is printed on your membership card. You may also obtain it by calling the county Farm Bureau office at (800) 451-8908.
• You may cast votes for at-large trustees, trustees for the district in which you reside, and delegates to the 2021 OFBF Annual Meeting. Any votes cast for districts other than the one in which you reside will not be counted.
• Each active member shall have one vote. In the case of married members, both husband and wife are entitled to one vote each.
• The names appearing on this pre-printed ballot are the candidates who have been nominated by the
   Nominating Committee appointed by the Board of Trustees. You may vote for one of the candidates listed,
   or write in the name of any other eligible member and cast your vote for that person.
• The Secretary shall disregard any proxies that are illegible or unclear.
• Proxy Ballots must be either (1) dropped off in-person, or (2) mailed to: Delaware County Farm Bureau, PO
   Box 368 Hilliard, OH 43026. Proxy Ballots may also be (3) scanned and emailed to: delaware@ofbf.org.
   Proxy Ballots must be received by 4:30 p.m. on Tuesday, September 1, 2020 to be counted.
• You may submit a Proxy Ballot whether or not you attend the Virtual Annual Meeting.
• If you submit a Proxy Ballot and then choose to attend the Virtual Annual Meeting, you may revoke your
   previously cast ballot by providing notice to the Secretary and cast your votes during the Virtual Annual
   Meeting.
I, the undersigned active member of the Delaware County Farm Bureau, hereby appoint the Secretary of the Delaware County Farm Bureau as my proxy to represent me at the Annual Meeting of the Delaware County Farm Bureau on September 1, 2020 and to cast my vote(s) pursuant to the instructions provided below for: (i) election of Trustees of the Delaware County Farm Bureau Board of Trustees, (ii) proposed policies and (iii) election of Delegates to the 2021 Ohio Farm Bureau Federation Annual Meeting.
