TOPP KIDS Program Awards: Community of Care & Innovative Programming
The following form should be used to nominate a Program for the following two Awards categories: Community of Care and Innovative Programming.

Program awards are self-nominated, meaning that this nomination form will be developed with the support of the team, and approved/submitted by the Program Coordinator. Only one nomination should come in per program for each program award category.

Programs may nominate their team for one or both of these two awards, but please submit only one copy of this form (if applying for both Community of Care AND Innovative Programming, please submit information for both on one form). For the nomination to be considered, you must respond to all three written responses within the nomination category.

Along with this nomination form, please provide all required documentation/evidence to demonstrate the award-specific outcomes and achievements.  Documentation can be submitted to info@toppkids.com. Your email's subject line should include the award you are nominating for and your program name (i.e. Community of Care - Prince of Wales). Please ensure that both this form AND your documentation has been received.

Nomination Closing Date: April 7th 2024

After the nominations closing date one designated member of the Support Team will compile the data collected for each Award category and randomize the information, removing any identifiable features.

A new form for each category will then be created, with criteria descriptions only and no names of nominees - that way voting is anonymous and fair, and votes will be cast not just for individuals based on their names, but based on the merit of the information provided about them in the Nomination forms. This year, the Support Team (except for the person who compiles the data) will complete the anonymous vote on April 15th. There will be no discussions, simply an individual assessment on the merit of each contribution and a vote. From there, votes are tallied and the winning descriptions will be matched with the name of the nominee, and awards winners will be determined in late April.
