This form is for current and former GitLab Team Members who wish to obtain personal data beyond what is included in a Personnel File. Please note that if you are were were employed through a PEO, GitLab may be unable to respond to your request and you should submit your access request directly to the PEO. For U.S. Team Members, only residents of California are eligible to obtain personal data outside what is included in a Personnel File.
How It Works:
Step 1 - Fill out this form so we can process your request.
Step 2 - We will contact you via email to confirm your request.
Step 3 - We will validate your request and confirm your identity to ensure we do not provide personal data to the wrong person.
Step 4 - Once we verify your identity and confirm we can fulfill your request, we will search our systems, create a report of the personal data collected and held about you within the agreed search parameters and email that to you.
We will fulfill this request within thirty (30) days of verification of your identity.
