Carpool Group:  Carpooling will be encouraged this year!  Please list each child's name in the carpool.  We will do our best to group your children on the same days.  We will assign which days your group will attend school on campus and will notify each parent.    
Siblings: My children have different last names and I want them to attend school on the same days. Please list each child's first and last name.  (We will assign which days your children will attend school on campus and will notify you.)
Other: Please list your child/children's first and last name, your request, and the reason for your request.  We will do our best to accommodate your request and will notify you of our decision.  
Wednesdays: I am requesting my child/children attend school on Wednesdays for extra support.  Please list each child's first and last name and the reason for the request. (My child receives EC services, is in the ESL program, is in the AIG program, etc.) We will do our best to accommodate your request and will notify you of our decision.
