There is a $30 fee per child to participate in Basketball Camp. This registration fee covers your child's participation in camp, snacks, a t-shirt, and a basketball.

By electronically submitting this form, you hereby give consent as the parent/guardian for the child/children listed above to participate in Good Shepherd Lutheran Church's Basketball Camp. You and your child/children hereby release, indemnify, and hold harmless the church and its employees and/or volunteers from any and all liability for any claim, injury, or loss sustained by or during your child's participation. You furthermore authorize Good Shepherd Lutheran Church to take and use photographs and/or video of your child for crafts, keepsakes, or promotional purposes in any type of media and understand you will not be compensated for any such use. In the event of an emergency, the volunteers have your permission to seek necessary medical treatment for your child/children.
PLEASE NOTE: Your registration fee is required to reserve your child's spot for basketball camp.

All of our payments (even credit card payments) are received via PayPal, but you do NOT need a PayPal account.
