Friday July 3 we will be having a Dodgebee tournament! If you're wondering what that is, thinking of a soft frisbee that you use like a dodgeball. You've got the picture! We'll have an epic tournament that will end in the winning teams receiving great prizes.

Where: Youth Outreach Center, 2 Holy Cross Path, Sai Wan Ho
When: Friday July 3, 2-6PM
Details: Youth are welcome to choose their teams in advance! Teams are limited to 8 players. If you are signing up with a team please fill out the questions below labeled "Team Name" and "Team Captain." If you are signing up without a team, don't worry! You'll be grouped with a team when you arrive.

I recognize that there are risks involved in participating in the event and hereby assume all risk of injury, harm, damage, or death in connection with my participation in it. I understand and agree that neither Union Church nor its Trustees, Deacons, CoM, directors, employees and volunteers may be held liable in any way for any injury, harm, damage, or death that may occur to me as a result of my participation in this event and hereby release Union Church, its Trustees, Deacons, CoM, directors, employees and volunteers from any injury, harm, damage or death, which may occur while I am participating in the event

I authorize Union Church through its Trustees, Deacons, CoM, directors, employees, volunteers to render or obtain such emergency medical care or treatment for me as may be necessary should any injury, harm or accident occur to me while participating in the event.

I understand and acknowledge that Union Church does not provide health or medical insurance in connection with the event and I agree that I will be financially responsible for any bills incurred as a result of medical treatment, including emergency medical treatment and/or transportation to a medical facility, in connection with my participation in the event
