Tadpole Kit Request form for Term 4 2023

Cost per kit: $100 ($50 refundable deposit and $50 loan fee)

1. Fill in this form for your Tadpole kit(s) - kindly sponsored by Icon Water - and submit  
     NOTE: ENTER YOUR SCHOOL'S BANK ACCOUNT DETAILS IF YOU WOULD LIKE YOUR DEPOSIT BACK

2. You will receive an invoice within 5 working days, including your payment reference and due date

3. Payments that are late or without reference number will not be accepted

Tadpole Kits will be ready for pick up on the first Monday of Term 4 = 15.10.2024
Tadpole Kits return is scheduled for the last Monday of Term 4 = 16.12.2024
