Let your child complete the questionnaire for assessment.  If the child can read and understand the requirements of the questions, he/she can answer the questions by himself/herself.  If your child is too young, parents can understand the questions first and then use a chatting way to induce the child to answer the questions in a relaxed atmosphere.
