DCE PTA Parent Volunteer and Class Roster/Directory Form - 4th Grade

This year, the PTA is creating a volunteer team and roster by grade instead of by class so we can send volunteers where they are needed most. We still want to support our teachers with appreciation and gifts plus be able to communicate with other parents to organize our efforts. If you choose to assist in a volunteer position, a virtual meeting will take place on 10/15 with further information. Please be on the lookout for an email with more information on the meeting.

Do you want to give permission to include the email address, phone number and home address in the whole grade roster and school directory? Previously each class had a class roster, but this year the roster will cover the whole grade. The roster serves as a way to communicate between the teacher and volunteers for the class. Deer Creek Elementary and LISD are not permitted to release your information to the PTA. The roster and school directory are coordinated through the DCE PTA and is not part of DCE or LISD. This information will be shared with only the other parents in the grade (roster) and school (directory) and should not be given out. This is your opportunity to be in the roster and directory. *

Room Rep - Be the main liaison between the PTA and the room rep team. Help coordinate the team to make sure all responsibilities (listed below) are covered. Work with the teacher to identify any other needs that need to filled for the class.

Teacher Appreciation - create a plan for the week to show appreciation from the class. The PTA will already have some activities planned. Send a message to parents asking for donations, if needed.

Teacher Birthday - coordinate something special for your classroom teacher. i.e. A gift, special message, lunch. Send a message to parents asking for donations, if needed.

Class Roster - Create an excel spreadsheet with the information given from the PTA. Distribute the roster via email to the parents listed on the roster.

Possible end of the year party - Help organize volunteers and activities for the class.

A copy of your responses will be emailed to the address you provided.
