I understand that camp activities at the Kids Clubhouse will involve some physical activities which carry with them certain inherent risks of personal injury. By allowing my child to participate in these activities, including on campus and field trips, I understand and accept the associated risks and I will be responsible for any injury or other loss which may occur during my child(ren)’s participation of these activities.

I understand that Kids Clubhouse officials will make every effort to contact the Parent/Guardian if my child requires emergency medical treatment. If such contact cannot be established, I hereby authorize Kids Clubhouse or RHCBC officials to sign consent form for medical treatment and to authorize any physician or hospital to provide medical treatment or procedures for my child(ren). I am responsible for any expenses associated with such treatment or procedures. Important note: RHCBC accepts no responsibility for refusal of care if Provincial health care information is not provided.

I agree that photographs, video or audio recordings of my child(ren) at the Kids Clubhouse may be taken by RHCBC, and that they may be used, published or distributed to other campers for promotion or other purposes. I waive all rights in these photographs or recordings and release RHCBC from any claims for libel and invasion of privacy.

I undertake and agree to indemnify and hold blameless RHCBC, its directors, officers, employees, members and volunteers from and against any loss, damage, injury or death suffered by my child as a result of participation at the Kids Clubhouse, as well as of any medical treatment authorized by the individual representing RHCBC.
