Tonka United Service Project Participants Form
Team Service Projects Coordinators complete this form to submit the names of players that participated in a service event with their team.  Players that participated will be recognized with a certificate at the end of the year.  This form will go to the Service Project Program Lead, Sarah Ostendorf.  Reach out to Sarah at sarah.k.hutchinson@gmail.com if you have any trouble.  Thank you for volunteering and helping your team give back to the community!

Enter the number of all the people that participated in the project including players, adults, siblings, coaches, etc.
Total # of Hours Delivered *
Estimate the total number of hours contributed to the community across all participants e.g. if 10 people spent 2 hours each, enter 20 hours.
First & Last Name of Players that Participated *
We will use this information to create the player certificates.  When entering names, press enter to get to the next line after each name
