This form is to get potential plasma donor information to receive a plasma transfusion to COVID -19 patient in Houston and not to overwhelm the patient family with the phone call. This information is directly received by a family member of the patient.

The following are the criteria the donor should meet.
1. The donor must be COVID-19 positive in the past (Not symptoms but actual positive test confirmation)
2. The donor should have been recovered from the COVID-19; he/she may have been tested for the negative test and should not have any symptoms of COVID-19.
3. Preferable Houston/Austin/Dallas as the donor should be able to come to Houston on an urgent basis.
4. Blood group A or AB, but please fill this out this form and let doctors determine if your blood group is other.
