ACTIon practitioner log
The ACTIon project is co-funded by the Erasmus+ Programme. This log will support you reflect on your implementation of DigiPAC or MOLA, and also provide evidence for quality assurance and evaluation of the project. The log should be completed after each ACTIon training session or series of sessions.

The data you submit will inform both internal evaluation (carried out by nexus - https://nexusinstitut.de) and external evaluation (carried out by Isham Education and Community). The information you provide here is for evaluation purposes only. Any information you provide may be reported anonymously in ACTIon reports only. Your information will not be passed to any organisation beyond the ACTIon partnership (https://www.erasmus-action.eu/partners/), and your name will never appear in any report.

You provide your name so we can understand impact at individual level. Your name will be deleted from the records of this evaluation at the end of the project – and by June 2024 at the latest.
