What is this form for?
The Uplawmoor Community Council, Uplawmoor Development Trust, and Caldwell Parish Church have started a project to provide a "safety net", ensuring that everyone in our community is able to get help during the coronavirus pandemic should they need it.

This form allows you to register as being available to help others within the village should they request assistance as a result of self-isolation.

It also allows local organisations (including businesses and community groups) to share what services they offer that may be of help to those in self-isolation.
Who is collecting this information?
Uplawmoor Community Council (UCC) is acting as data controller, and will manage and store the information received. The Uplawmoor Development Trust and Caldwell Parish Church are data processors under the meaning of the Data Protection Act. For more information, please contact secretary@uplawmoorcouncil.co.uk
What we will do with the information you provide?
If you are submitting this as an individual: we have launched a dedicated phone number and email address to receive requests for assistance from those in the community. When we receive a request that we believe you could meet, we will contact you to see if you are happy to do so. Your information will not be distributed publicly at any point, and will be deleted when the self-isolating restrictions end, or sooner if you ask us to.

It's a little different if you are an organisation. If you are an organisation and the service you offer may be of particular help to those self-isolating, we may share the information you provide on our website and social media, or in regular bulletins we produce.
What else can I do?
This project is to provide a final "safety net" for those who are unable to get help from family and friends. If you feel happy to do so, please also offer help to your neighbours directly. We're here to make sure no-one is completely isolated; ideally we will prevent anyone from ever reaching the point.
