Lifeguard Mather 2023 Qualifications
This form will allow you to give us your lifeguard experience. THIS IS FOR CAMP MATHER 2023 Summer Jobs - meaning that you must be willing to work & live in shared housing for at least a twelve week period of time - Camp Mather is approximately four (4) hours (180 miles) away from San Francisco   NOTE - you must have a current Lifeguard/First Aid/CPR certificate AND at least 200 hours of qualifying lifeguard experience in order to qualify for this job.

Vaccination Status - the City & County of San Francisco requires all employees to be fully vaccinated (14 days after your final dosage).  In order to ensure we are using everyone's time efficiently, we need to ask you about your vaccination status now and if we offer you the job, we will require you to submit the documentation. *
Must be 18 years of age -   I understand that I must be at least 18 years of age in order to work at Camp Mather. I am certifying that my year of birth is within 2004/5 or before 2004/5 *

Date of Arrival - If you are hired, please give us an anticipated date of being able to come to Camp Mather (4 hours away from SF) It must occur between May 18/19  and June 1.  I will commit to at least twelve (12 weeks) until August 25 *
Getting to Camp Mather  - NOTE - you are responsible for arranging your transportation to and from Camp Mather - it is approximately 180 miles from San Francisco.  Possible transportation methods include personal vehicle, family member driving you to and from camp, carpooling with others to camp, flights to the Bay Area if out of state, etc. For your reference, the address to Camp Mather is: 35250 Mather Road, Groveland, California 95321.  Please document that you understand this *
Time off - you probably will have split days off at Camp Mather.  If there is a chance you are already planning a trip, please indicate the dates in advance here so we can take that in consideration   *
